Orientation

If you are unable to attend the scheduled Orientation before you start the treatment process, you can complete it on your own.

Directions:

1) Download the following forms (Note: Each link will open the file in a new window):

Informed Consent

Release of Information (Place in your information on the first line and the information for your attorney or probation officer on the other lines). Note: You may need to fill out more than one release if more than one person needs verification of your treatment.

Financial Contract (You must mark the program that your evaluation recommended.) Note: Cost will be determined based upon:

Abstinence Contract

2) Fill out each form.

3) Download and print off the Orientation Worksheet.

4) Watch the Orientation Video on our YouTube page and fill in the answers as you go.

5) Bring all of this paperwork to your first session.
